These Terms of Service ("Terms") form a binding agreement between you and GO TEAM HC ("GO TEAM HC," "we," "us," or "our"), a solo family medicine practice led by Dr. Bethany E. Powell, MD, located at 1802 Broadway St, Suite #228, Galveston, TX 77550-4913. By accessing our website, patient portal, or scheduling an appointment, you agree to be bound by these Terms. If you do not agree, please do not use the services.

You must be at least 18 years old (or the age of majority in your jurisdiction) to create an account. You are responsible for maintaining the confidentiality of your login credentials and for all activity that occurs under your account. Appointment requests submitted through the site are not confirmed until our team responds and schedules the visit. Please provide at least 24 hours' notice for cancellations. Fees, insurance acceptance, and payment terms are communicated at the time of scheduling and may change from time to time.

We may suspend or terminate access to the services at any time for conduct that we reasonably believe violates these Terms or is harmful to other users, to us, or to third parties. Upon termination, the provisions that by their nature should survive will remain in effect.

These Terms are governed by the laws of the State of Texas, without regard to its conflict of laws principles. Exclusive venue for any dispute lies in the state or federal courts located in Galveston County, Texas.

We may update these Terms from time to time. When we do, we will revise the "Last updated" date above. Continued use of the services after changes take effect constitutes acceptance of the revised Terms.
